The network name and/or logo should be shown somewhere on your ID card. If you do not find it, you can call the Customer Service number on your card to inquire.

You may also want to call your Benefits Dept. and ask how allergy testing is covered. It is sometimes covered differently than the office visit. Some policies have limitations on the number of skin tests you can have per year.

Due to privacy laws, we cannot contact your insurance company on your behalf without specific information and permission. Should you wish us to do so before you have been seen in our office, you will need to complete a "Permission To Discuss PHI" form. Please print the form, complete it, and fax to 979-776-4260.
